Position Summary:
The Consumer Community Support Partner (CSP) provides direct, hands-on support to individuals enrolled in TDC programs, assisting with daily living, personal development, social engagement, and community involvement. This role is essential in helping participants (consumers) achieve greater independence, dignity, and inclusion through compassionate and personalized care.
CSPs must be dependable, empathetic, and committed to delivering respectful assistance, which may include personal hygiene support.
Key Responsibilities:
Daily Support & Engagement:
- Provide one-on-one and small group support in accordance with each consumer’s individual support plan.
- Assist with daily living activities such as meal assistance, mobility support, hygiene reminders, and toileting as needed.
- Deliver compassionate personal care, including help with dressing, grooming, and in some cases, changing and cleaning individuals who require full support with toileting.
- Encourage active participation in structured programs and promote positive peer interactions.
Community Involvement:
- Accompany and assist consumers during community outings and events.
- Support individuals in navigating public settings safely and confidently.
- Foster inclusion and independence during volunteer opportunities, recreational activities, or field trips.
Skill-Building & Personal Development:
- Help consumers build and practice essential life skills, such as communication, time management, money handling, and goal setting.
- Reinforce progress toward individualized goals through daily interaction and encouragement.
- Support consumer self-advocacy and personal growth.
Team Collaboration & Documentation:
- Record daily notes, incident reports, and goal tracking accurately and in a timely manner.
- Communicate with supervisors and interdisciplinary team members to support continuity of care.
- Participate in team meetings, trainings, and professional development as required.

- Maintain a safe, respectful, and inclusive environment at all times.
- Ensure all duties, including personal care, are performed with dignity, discretion, and professionalism.
- Follow all TDC protocols, including health, safety, and confidentiality standards.
- Remain flexible to adapt to changes in consumer needs, program schedules, or support assignments.
- Be available for evening/weekend special events when needed.
Preferred Qualifications:
- Experience working with individuals with intellectual and/or developmental disabilities (preferred but not required)
- Strong interpersonal, problem-solving, and communication skills
- Ability to manage challenging behaviors with patience and professionalism
- CPR/First Aid certification (or willingness to obtain)
- Valid driver's license and reliable transportation (if assigned to off-site support)

CSPs will work in a variety of settings including program centers, community spaces, and occasionally in consumers’ homes or public venues.
